Name the supplier, list what you are ordering at the agreed prices, and say where and when it is needed. The order is drawn as you type and downloads as a PDF.

Raising the same orders to the same suppliers every month?
Keep them in a spreadsheet and BroadPaper Cloud makes a purchase order for each supplier at once, with your letterhead and the right delivery address on every one.

Questions
Is a purchase order legally binding?
Once the supplier accepts it, it normally forms the contract for that order on the terms it states. Put anything that matters — price, delivery date, payment terms — on the order itself.
Should the prices on a purchase order include VAT?
Show the prices as agreed with the supplier, and the VAT rate if they are VAT registered. The order shows the VAT separately so the total is clear.
Is this purchase order template free?
Yes, with no account, and nothing you type is uploaded. The PDF carries a small BroadPaper line at the foot, which BroadPaper Cloud's paid plan removes.
